Is Online ADHD Assessment Valid? What the Research Says
When it comes to something as significant as an ADHD assessment, it’s natural to have questions. One of the most common concerns we hear is: "Is a telehealth assessment really as valid as seeing a psychologist in person?"
For a long time, the clinical interview in a physical office was considered the only way. But the world—and the science—has moved on. If you are a Support Coordinator looking for a client, or an individual seeking clarity, here is what the latest peer-reviewed research from the last five years (2021–2026) tells us about the truth of online ADHD assessments.
What the Research Says
Recent studies have put online ADHD diagnostic pathways under the microscope, and the results are encouraging.
High Accuracy: A 2025 study published in the Journal of Clinical Psychiatry by Herman et al. evaluated real-world online ADHD assessments and found a high positive predictive value of approximately 95%. This means that when an online assessment identifies ADHD, it is highly likely to be accurate. Read the study summary
A Conservative Approach: Interestingly, research suggests that online tools and psychologists working via telehealth are often more conservative than in-person interviews. The same 2025 study noted that disagreements between online screeners and full clinical interviews usually resulted in the online tool being more cautious to avoid overdiagnosis. View the article
Reliability: Scoping reviews such as Susmarini et al. (2024) have found that while some computerized testing still requires refinement, the core of an ADHD diagnosis—structured interviews, behavioral checklists, and history taking—can be delivered reliably through telehealth when clinicians use appropriate protocols and secure video platforms. Why Telehealth Can Be "Better" for Neurodivergent Brains
Beyond just being "as good as" in-person, telehealth offers specific advantages for the ADHD brain:
1. Sensory Control
A 2024 survey by Hayden found that while some parents worried about distraction, many also valued the comfort and practicality of home-based telehealth. For adults with ADHD, being in a familiar environment can reduce some of the sensory load that comes with attending a clinic in person. Read the study
2. Removing the "Travel Tax"
Navigating traffic, finding parking, and arriving on time can create genuine executive functioning hurdles. Telehealth removes much of that stress, so the person arrives at the session with less transition strain and fewer logistical barriers.
3. Accessibility
For people in rural Australia, those with limited mobility, or those trying to find a clinician with experience working with neurodivergent clients online, telehealth can make access far more realistic. It also supports referral pathways for Support Coordinators and therapy managers looking for appropriate options for clients.
A Balanced View
Is telehealth always the answer? Not necessarily. If there are complex physical comorbidities or a need for hands-on neuropsychological testing that has not been validated for remote use, an in-person follow-up may be recommended.
However, for the vast majority of adults seeking an ADHD assessment, the evidence is clear: telehealth is a robust, evidence-based, and valid choice.
